PHOTO PERMISSION: I give permission for SHE Getaways and Fall Fit Women's weekend to use any pictures of me for social media and/or promotional purposes. MEDICAL TREATMENT: I hereby give permission to be given First Aid and CPR by a qualified staff member. I give permission to be transported by ambulance or aid car to an emergency centre for treatment. I further consent to disclosure of health information and to the medical, surgical, and hospital care treatment and procedures to be performed by a licenced physician or hospital when determined immediately necessary or advisable by the physician to safeguard my health. RELEASE FROM LIABILITY: Recognizing that SHE Getaways Fall Fit Women's Weekend will do its best to ensure a safe experience, I understand that accidents may occur from both my participation in SHE Getaways Fall Fit Women's Weekend and from transportation to and from the program. I agree to assume these risks. By directors, officers, and agents from liability based on any damage, loss or injury whether it is a result of ordinary negligence, or otherwise, caused to me from participation in this program. REFUNDS: I understand the refund policy of SHE Getaways Fall Fit Women's Weekend. My spot is 100% transferrable to any woman I find (SHE Getaways strictly cannot facilitate transfers) up to 48 hours before the program starts. In this case, I will collect the registration monies from her directly and contact SHE Getaways to advise of her name and email address. If I cancel my registration before August 15th, 2020 I will receive a full refund minus the $150 non refundable deposit. There are no refunds whatsoever after August 15, 2020. BUNK BOOKING: I understand that after this registration is complete, I am required to book my bunk with an email transfer of $150 to [email protected] within 72 hours. Failure to do so may result in me losing my spot at Fall Fit should it sell out. I have read and agree to these terms as indicated by my signature below.
